  • Validation of a novel low-fidelity microvascular anastomosis teaching model: Eda hospital experience

    Purpose:
    Under This prospective study evaluated the novel microvascular anastomosis model: E-DA hospital model (EDAM).

    Materials and Methods:
    Participants include microsurgeons, experienced trainees, and novices. Microsurgeons test the EDAM and evaluate the educational value scores and the performance of experienced trainees and novices. Experienced trainees and novices attended the one-day microvascular anastomosis course. They were asked to fill the microvascular self-confidence test before and after the course. They practiced anastomosis on latex gloves, EDAM then the dorsal vessel of a chicken foot. Stanford Microsurgery and Resident Training scale (SMaRT) and leakage test were used to evaluate the learning curve of participants.
    EDAM was made of plastic cup, latex gloves and inserted intravenous drip. The latex gloves covered the plastic cup and were extended to simulate the touch of vessel wall. The height of intravenous drip was adjusted to keep the pressure of cup around 30-40mmHg, which is the pressure of peripheral microvascular systems. Participants were asked to suture the 5mm cut in the center of gloves. Results were analyzed with ANOVA and Student t test, with P less than 0.05 indicating statistical significance.

    Results:
    3 Microsurgeons, 10 experienced trainees and 24 novices were involved. The microsurgeons evaluate high educational values score of the EDAM. More experienced trainees tended to get higher SMaRT score on EDAM from less experienced novices. Participants who viewed themselves as being confident received a higher score on the SMaRT scale. Also, those who got higher scores on EDAM also tended to get higher SMaRT on anastomosis on vessel of chicken foot. There were no significant differences in scoring among all 3 microsurgeons evaluators.
    Conclusions:
    The EDAM was showed to be a cost effective with high accessibility microvascular training device.
