  • Multidisciplinary Management of Complex Pediatric Traumatic Orthopedic Injuries Resulting in Chronic Osteomyelitis and Non-healing Soft Tissue Complications: A Case Report

    Objective:
    This study delves into the intricate case of an 8-year-old girl who endured a life-altering motorcycle accident, experiencing severe orthopedic injuries with cascading complications. The primary objective is to comprehensively elucidate the diverse array of treatment modalities employed, expound upon the multifaceted outcomes, and elucidate the formidable challenges encountered during the course of her care.
    Materials and Methods:
    The patient, a young girl of 8, became a victim of a harrowing motorcycle accident in March 2021. The collision exacted a heavy toll on her body, inflicting complex injuries that included open fractures of her left femur and tibiafibula. This devastating incident also instigated popliteal artery traumatic dissection with subsequent thrombosis and an associated bout of compartment syndrome. In response to this medical emergency, an extensive series of surgical interventions was initiated. These procedures encompassed the intricate realm of muscle and tendon repairs, as well as the arduous and often multifaceted process of wound debridement. Concurrently, the clinical team employed negative pressure wound therapy in their relentless bid to ameliorate the patient's condition. As the course of the patient's care unfolded, a relentless and intractable chronic non-healing pressure ulcer manifested on the left foot. The unyielding nature of this wound was underscored by the unnerving exposure of the fibular bone, culminating in a series of painstaking surgeries. These procedures included split-thickness skin grafts, judicious use of biodegradable temporizing matrices, and the application of Integra coverage, all accompanied by the indispensable support of negative pressure wound therapy.
    Results:
    The patient's journey through the corridors of medical intervention was marred by the ceaseless and formidable challenges that she had to confront. The constellation of complications that punctuated her progress included the emergence of left leg polyneuropathy, scar contracture, and the development of tenacious ankle adhesions. In an attempt to orchestrate an effective response to these adversities, the clinical team instituted a multidisciplinary approach. This approach, by its very nature, necessitated intricate consultations with the vaunted orthopedic specialists whose wisdom and expertise in musculoskeletal health were brought to bear. It also summoned infectious disease experts to the fore, offering insights and expertise in combating recalcitrant infections and preventing their resurgence. In the shadows, yet an essential component of the multidisciplinary ensemble, the rehabilitation team played an indispensable role in orchestrating and monitoring the patient's therapeutic journey.
    Conclusions:
    This case illustrates the complexities of managing pediatric patients with severe traumatic injuries and subsequent complications. It emphasizes the important role of a multidisciplinary approach involving orthopedic, infectious disease, and rehabilitation specialists. Continuous monitoring and timely intervention are crucial, especially in cases of non-healing wounds and potential development of chronic osteomyelitis.
