The prevalence of breast cancer is gradually increasing. Post-mastectomy breast reconstruction can improve the appearance of the breast. Autologous tissue-based breast reconstruction has been associated with higher postoperative satisfaction and psychosocial well-being according to previous literature. However, it may have some complications to compromise the surgical outcomes, such as vascular thrombosis, flap necrosis and donor site morbidities. The aim of this presentation is to review and analyze the clinical outcomes at Taipei Veterans General Hospital. By means of donor site selection, perforator mapping, volume estimation, flap shaping and even flap prefabrication by cell therapy, we could minimize complications and achieve optimal outcomes
