  • The effect of treating complicated skin and soft tissue infections with the concept of "mini-IDF"

    Complicated skin and soft tissue infections (cSSTI) including abscesses, cellulitis, fasciitis, diabetic foot infections and post-trauma and surgical site infection are among the most common infections treated in the hospital setting. cSSTI are reported to account for up to 10% of admissions to infection units in the USA and in the United Kingdom. Patients frequently receive broad spectrum intravenous antibiotic therapy and intensive surgical intervention, and treatment may be further complicated by the presence of significant co-morbidities.
    From October 2021 to September 2022, a retrospective chart review study was conducted at Kaohsiung Hospital, Taiwan. Patients with cSSTI underwent surgery with the concept of "mini-IDF" were included. Characteristics of patients were collected and discussed.
    Good management of persisted inflammation remains the top priority in the treatment of cSSTI wounds. Good preservation of skin envelops provides options of simple reconstructive surgeries for plastic surgeons. In this study, pain reduction, skin integrity and wound promotion were improved in patients with skin and soft tissue infections. We revised the guideline for the management of diabetic foot ulcers "VIPDF" introduced by Dr. Shigen Chen to "mini-IDF", that is, minimally invasive surgery, I-infection and inflammation control, D-debridement, F-Fespixon cream.
    We share our experience with the combined use of antibiotics and Fespixon in cSSTI wound management.
