Open thyroid cartilage reduction is currently the standard surgery option for the neck feminization surgery. However, this approach leaves an unavoidably visible neck scar especially for Asian patients. In addition, changes made to the forehead and periorbital region can have significant effects in gender-affirmation surgery. Removal of frontal bossing and alteration of orbital shape can result in significant facial feminization. We will provide our experiences of scarless thyroid cartilage reduction and discuss the indications, surgical procedures, outcomes, and possible complications for the forehead set-back surgery.
