  • Two cases of hand dermatofibrosarcoma protuberans reconstructed with SCIP flap

    Objective
    Dermatofibrosarcoma protuberans (DFSP) is a slow growing, low- to intermediate-grade dermal soft-tissue tumor with high local recurrence rate but low metastatic potential. The standard treatment is complete surgical excision. These two patients had hand dermatofibrosarcoma protuberans at dorsal and ventral side of hand respectively. Our aim is to make clear resection margin of tumor and reconstruct the defect with good preservation of hand function.

    Material and Methods
    Case 1
    A 34-year-old woman with underlying disease of morbid obesity status post laparoscopic gastric bypass in 2014 had recurrent dermatofibroma at left dorsal hand six months after tumor excision. There was no distal metastasis. She received operation of wide excision with safe margin of 3 centimeter and the base was resected until tendon layer. The defect was later reconstructed with left free SCIP flap under ICG image assistant and the whole flap showed good enhancement.
    Case 2
    A 23-year-old woman without systemic disease hand left palm dermatofibrosarcoma at metacarpal joint level between middle and ring fingers. She had wide excision with safe margin of 3 centimeter and received reconstruction with lambda shape designed SCIP flap.

    Results
    We had successful reconstruction in both patient with dermatofibrosarcoma of dorsal hand and palm. These two cases not only had been free of the tumor but preserved good function of hands in both motion of flexion and extension. The SCIP flap survived well and also showed good aesthetic results.

    Conclusions
    The SCIP flap is a technically challenging but versatile, thin flap with minimal donor site morbidity that can be used to resurface the defect of hand and preserve good hand function in the same while. The SCIP flap is indeed a valuable resource for the hand surgeon confronted with soft tissue defects of the hand.
