  • Clinical application of SVF-gel: Facial augmentation and skin rejuvenation

    Autologous fat grafting contains numerous adipose-derived stem cells, which is applied for revitalization and harmonization. In recent years, several mechanical processing procedures, including centrifugation, mechanical chopping, shredding, pureeing, and mincing, was used to obtain adipose stem cells without collagenase involved.
    Based on this concept, we developed SVF-gel, an injectable mixture generated by a simple mechanical process and particularly rich in ASCs, vascular endothelial cells (ECs), and native adipose extracellular matrix (ECM). The product of this protocol was defined as SVF-gel if 1) its final volume was < 20% of the initial volume; 2) the SVF cell density was >4.0×105 cells/ml; and 3) it was injectable through a 27-gauge needle.
    Based on our clinical application, SVF-gel showed great potential in facial volumization and skin rejuvenation. SVF-gel grafting for skin rejuvenation and facial volume augmentation was tested in patients. This product showed extraordinary results in treatments for periorbital hallow, neck horizontal wrinkle and chin augmentation. To our surprise, SVF-gel transplantation had a better retention rate than Coleman fat.
    In our animal model of SVF-gel transplantation, we discovered that SVF-gel has a high long-term retention rate and a unique adipose regeneration mode, involving prompt inflammation and infiltration of immune cells, stimulating rapid angiogenesis and inducing host-cell-mediated adipogenesis.
