The Pectoralis Major myocutaneous flap (PMMC flap) which was developed by Arian 1979 has some requirements (safety due to the pedicle flap and its wide muscle body, short operation time, less invasiveness , etc. ) in the head and neck reconstruction in spite of the recent development of the free flaps. Thus, PMMC flap is not an old flap. On the other hand, it has some problems (unstable blood circulation, restricted range and flexibility, etc.). So, we have tried to solve these bloblerms and developed a new type of the PMMC flap. In this lecture, I’ll make a presentation about the efficacy of the PMMC flap including the new one for the head and neck reconstruction, especialy in the trouble eases and the hard neck cases.
