  • Joy and sorrow of rhinoplasty with DCF

The nose of Asians has a low dorsum, so implants such as silicone and gore-tex have been used a lot for augmentation of the dorsum. However, various complications caused by implants have been reported. So, recently, rhinoplasty using autologous tissue is increasing. Materials such as autologous dermis, autologous cartilage, and fascia are used as materials for augmentation rhinoplasty. In the case of autologous costal cartilage, it is an excellent material for augmentation rhinoplasty, but there are problems with warping, visible problems, and the need to harvest cartilage as long as the length of the dorsum. In the case of autologous dermis, there is a problem of high absorption rate and scar of donor site. In the case of Diced cartilage wrapped in fascia (DCF), long-length cartilage is not required, and surgery can be performed using various cartilage. In addition, the absorption rate is low, making it suitable as a material for Asian nose shaping. However, in the case of DCF, there are also problems such as asymmetry and irregularity, and delicate manipulation is required to overcome this. The author would like to report on the merits and issues to be improved through 93 cases of experience over the past 8 years.
