Medial sural artery perforator flap is considered to be the one of best option for defects requiring thin and pliable coverage. However, the higher failure rate precludes many surgeons to use this flap as a first option. This study not only focuses on increasing the reliability of the flap but also on expanding its versatility to cover even the middle third leg defects.
